Dana Traxler, BBA, RN Executive Director
With over 20 years experience in Home Health Care, Dana was instrumental in developing the Visiting Nurse Association in the Mid-Ohio region. The VNAMO office opened in January of 2000, with a staff of 1.5 to over 100 dedicated professionals in 10 years. She has remained focused on finding better ways to provide home health services, lead and strategize, improve quality and safety, increase capacity, all while controlling costs. Dana received her nursing degree in 1989 and continued on to receive her Bachelor in Business Administration in 1999 from Mt. Vernon Nazarene College.
Erin Stucky, RN Associate Director
Erin oversees the day to day operations of Mid-Ohio. She has been an RN for 19 years and has been employed by the VNA since 2003. Erin has served in many different roles from a field staff nurse to Clinical Manager to now as the Associate Director. In 2008 The Distinguished Leaders in Health Care Committee awarded Erin the Mayor Lydia Reid Award of Excellence.
Rebecca Allison, RN, COS-C, HCS-D Quality Assurance Manager
Becky is responsible for overseeing the clinical practices at Mid-Ohio to assure we deliver care that meets VNA and as well as regulatory standards. She is a certified homecare coder and OASIS specialist and has been a nurse for over 30 years practicing in a variety of settings, but has always felt homecare nursing to be the most rewarding. Becky sees her predominant role as equipping the nurses with the education and tools to allow them to do what they do best: deliver excellent nursing care and case management to assist our clients in meeting healthcare challenges and goals and remain in their home.
Stephanie Taddeo, LISW Manager Mental Health
Stephanie has been with the Visiting Nurse Association for over six years. As Manager, Mental Health Services she promotes our home health mental health program by educating individuals and organizations about the services we offer and the affects of mental health issues on populations and communities. She supervises mental health nurses and social workers, and advocates for the consumers of mental health services. She received her BA from Miami University and Masters in Social Work from The Ohio State University with an emphasis in Health Care. She has over sixteen years experience in home based care serving diverse populations including those with substance abuse issues, HIV/AIDS, mental illness, bereavement and terminal illness.
Jennifer Hale RN, WCC Manager Community Care Ashland County
As Manager of Community Care for Ashland and Huron counties, Jennifer is responsible for helping VNA patients’ transition from Skilled Nursing Facilities or Hospitals to home with as little anxiety as possible. She meets with patients and families prior to VNA admissions to explain services VNA offers. She also helps monitor wounds and completes wound evaluations per orders. She makes recommendations to doctors, which help to have better outcomes for all of VNA’s wound patients. Jennifer has more than 17 years of nursing experience including long-term care, critical care and director of wound care in a LTAC hospital. She earned an Associate Degree in Nursing from North Central State College in Mansfield, Ohio and a Certificate in Wound Care from the National Alliance of Wound Care in 2006.
Trae Turner, RN Manager Community Care Richland County
Trae has been with the Visiting Nurse Association since October 2008. As Manager Community Care Richland County, Trae coordinates services for patients who are establishing home health care. He meets with patients, families, and facilities to ensure a patient’s transition home goes as smoothly as possible. Trae received his BSN from MedCentral College of Nursing in 2006 and has experience in medical/surgical and ICU nursing prior to his work with VNA.
Ashley Smith, RN Manager Community Care Crawford County
In 2008 VNA opened its Crawford County office with Ashley as Manager of Community Care. Ashley has worked in the home health field for 9 years serving patients in the community by providing direct care and facilitating home based services for patients. She isa member of the Human Service Council in Crawford County, The Chamber of Commerce, and serves as a member of the VNA Customer Service Committee. Ashley graduated with her RN, BSN from the University of Toledo/Medical College of Ohio in 1997.
Elizabeth Fitzpatrick, RN Manager Community Care Knox County
Beth a registered nurse is the Community Care Coordinator Manager of Knox County. Beth offers timely coordinated services for patients requiring care in the home setting and is especially trained to facilitate home care services, as well as educate patients and their families. Beth has been delivering professional assessments,coordination, and education to patients in home care for over 9 years. Beth is the recipient of the Press Ganey Customer Service Award, a Telecare Championship Nurse, sat on the CHF committee, and developed and serviced a Heart Failure Home Health Care Program for a local community in 2003. As a dedicate professional, she has recently helped to develop a Urinary Incontinence Management Program for the VNA. She holds an Associate Degree of Applied Science in Nursing from Marion Technical College and has been currently employed with VNA since 2007.
