VNA Hospice and Palliative Care

What is Hospice care? Hospice care is a special concept of care designed to provide comfort and support to patients and their families when a life-limiting illness no longer responds to cure-oriented treatments.  Hospice neither prolongs life nor hastens death.

Hospice care is for patients who have a life expectancy of months or weeks and wish to have treatment of pain and symptoms (palliative care) rather than curative treatment. The goal of hospice care is to improve the quality of a loved one's days by offering comfort and dignity with a special emphasis on controlling pain, symptoms and discomfort, and to make every day count for the patient as well as the family.   Hospice care focus is not just on the physical needs of patients and their families.

VNA Hospice care is provided by a team of specially-trained professionals and volunteers who not only focus on the physical needs of the patient, but also the emotional, social and spiritual impact of the disease on the patient, the patient's family and friends.  This is accomplished by offering a variety of bereavement and counseling services to better understand and manage the disease and accompanying grief processes before, during and after a patient expires.

The VNA Hospice team of professionals consists of Hospice-certified nurses, physicians, therapists, social workers, counselors, hospice care aides and volunteers who work together with patients, caregivers, families and personal physicians to provide the highest quality support,  comfort and communication.  An individualized  plan for care is developed for each patient and is updated routinely as the patient’s condition changes.

VNA of Ohio Hospice services include:

  • Ongoing medical care oversight and coordination
  • Pain and symptom management
  • 24-hour availability
  • Patient and caregiver education
  • Social Work services and referral to community resources
  • Hospice-approved transportation
  • Specialty hospice services for patients with Alzheimer’s/Dementia
  • Bereavement and spiritual care
  • Art and Massage therapy
  • Hospice-certified nurses
  • Coverage for hospice-approved medications, treatments and medical equipment

VNA Hospice promotes comfort and self-determination, enabling patients to participate in making decisions about care near the end of their lives. Our program is licensed and certified and offers crisis support 24 hours a day, 7 days a week. 

VNA also offers a pre-Hospice Palliative Care program, for patients with life-threatening illnesses who choose to continue to receive treatment for their illness.

Hospice care is a covered benefit under Medicare for patients with a prognosis of six months or less.  A patient may remain in hospice care beyond six months when a physician confirms that the patient is terminally ill.
